Cathy Smith empezó en el oficio de groupie a los 16 años con Levon Helm, batería del grupo The Band, quien le presentó a Rick Danko y Richard Manuel, bajista y cantante, respectivamente, del mismo grupo. Cuando se quedó embarazada, no supo cuál de ellos era el padre de su bebé, pero Richard Manuel, caballerosamente, se ofreció a casarse con ella para dar un padre al niño, que fue apodado como "el bebé de la banda". Sin embargo, Cathy rechazó la oferta. Luego fue amante del cantante y compositor canadiense Gordon Lightfoot durante 3 años, en los que los celos, las drogas y el alcohol fueron su día a día. Con el tiempo, Cathy terminó suministrado drogas a sus amigos músicos, como a John Belushi, al que inyectó el 5 de marzo de 1982 una dosis letal de heroína y cocaína, porque a él le daban miedo las agujas. Aunque en un principio la muerte de Belushi fue declarada accidental, Cathy Smith sería acusada de asesinato en primer grado, después de decir a la prensa lo siguiente: "Yo maté a John Belushi. No quise hacerlo, pero soy responsable". Finalmente, y tras alcanzar un acuerdo con el fiscal, fue condenada a 3 años de prisión por homicidio involuntario.
